Need Help

Every time I hit shift-D to add more mesh the mesh becomes all wrinkled and ugly and no matter how much I smooth it out the wrinkles never go away its a real pain in the ass to model with everything looking messed up.
Any help would be great thanks alot!

take a look at the wire in mud… think there are some tris and ngons in the mesh…
