centre insert cylinder in sphere?

Hi Folks
Could someone please shed some light on how to centre the insert cylinder brush on a sphere in Dynamesh. When I drag the insert cylinder, even using the shift key the cylinder it is always off centre in the sphere. I would like the cylinder to be perpendicular to the sphere in one axis either x,y or z. Is there a way to snap the cylinder to a axis inline with the spheres axis?
Or perhaps a tutorial or just answer confirming that it can’t be done.

Just guessing, but do you have local sym turned on for the main subtool?
