The only way I’ve been able to do that is to “liquify” or “warp” the motion blur once it’s done.
After Effects can do this and I’m sure there’s an action or script that might be able to help you in Photoshop, I’m looking right now as it has been something myself and others have needed.
Another option would be to create a brush from the image or element that you are trying to blur and use it - of course the brush will be one color so this may not work for an image that has several colors.

yes AE has many features that would be nice in PS.
the wrap thing can work - but is far from the optimal solution…

My sisters and I are working on a solution - honestly I’m surprized that no one has done this. I checked several PS scripting forums and sites and didn’t find anything close. I’m not sure how long it will take to create but I’ll keep you updated.
This is a frequent request, but the PS team has said that there’s a great deal of difficulty in doing this on filled layer. To be honest, I’m not really sure why but they have even been reluctant to make the current motion blur go only one direction, rather than spreading out along a line.
One way to work around this is to apply blur selectively on additional layers with various blend modes… make your selection, give it a new layer, and use the smudge and liquefy tools to rough in the motion blur you need. Then go back and get creative with the lens blur, add some noise, and finish it off with a texture brush on low strength to paint the blur on.
Yeah, it take a lot of work, but it gives really good results.

Not trying to sell Painter, but you can apply brushstrokes to a path, creating a seperate layer as the stroke is laid down. You can also capture an object for a brush stroke and effect fadeout and whatever you like. At the very least, try the trial download of Painter. Fully functional for 30 days for free. Painter can open and save psd files.
As far as I know, there is no solution for Photoshop. In After Effects you can use the SmoothKit.
