You can make 3ds Max plugins using maxscript.
Go through the topic ‘scripted plugins’ in the maxscript help.

In case you referred to the more hardcore c++ thingy, then max sdk is the least you need
in addition to visual studio.

You can’t, but it does come with the 3DS Max install DVD… (you have to install it seperately from the main app)
