How would you approach this?

Alright guys. I've been using Zbrush for a while. And I like to think that I know a lot of different approaches to my sculpts. But a friend wants me to sculpt his drawing for him and I cannot figure out a good way to approach it. I just keep running into problems. Sculpting the face around those tentacles is very challenging. I have tried appending them as subtools and then merging but then there is a very visible seam on the mesh where the subtool meets the base mesh. I have tried dynameshing my base head, and using curve tubes and then dynameshing the tentacles to the face, but then any major changes messes them up or I end up sculpting on them or smoothing them and ruining them. I am just a bit lost.

So my question is. How would you approach this? Or how would you recommend that I approach this?
