how to mirror sculpt on a unsymmetrical object ?

Is there a way to paint/sculpt symmetrically or mirror on a unsymmetrical object ?

The problem I face is the head of my object has shifted and no longer perfectly intersects the Y axis. I think their is or was a way to get Zbrush to do some smart calculations on geometry and still allow me to mirror paint on my object. Can Zbrush do this?

if it shifted on you, tool>deformation >unify
if you move the object with transpose, you will throw the sym out of whack
