Split up model into polygons

I have a super low-res model I want to break up into individual polygons (for a Fracture object).

Is there an *easy* way to do this? (other than manually selecting and separating every poly)

The Disconnect Command (Preserve Groups unchecked) does that.

You can also make a Explosion Deformer a child of your geometry, set the Explosion's strength to 0.001%, then drop your geometry into a Fracture Object (set to explode segments).

It will break your phong, but if you are going for the low-poly look, this may do the trick.
