perspective on the fly?

I have been reading a ton about perspective getting into some pretty advanced stuff much further than I have been before. I am just wondering do you experienced artists always plan out your perspective using lines and a seperate layer, or are you just so good at it you can do it on the fly? I am speaking mostly of environments atm.

When I'm just sketching, I usually eyeball the vanishing point, or maybe quickly draw a few lines directly on the sketch by hand to set a few rough guidelines. Once I'm ready to take the sketch into finished territory, I'd do a separate perspective guide with clean and straight lines on a separate layer (always in a distinct color so it's easy to see--something like non-photo blue or orange).
