One of the best things I've seen so far in CG.
This is real art, in my opinion. Realism bended to serve a fantastic concept: awesome.

great idea and execution. like the models and also textures looks really good.

for the shading, i think this could be improved. the sss for example, i think its hard to say that u used it (i would say you did not ). with 1 strong keylight and the other one you would expect way more of it.
skin of old people is very thin and sss is visible alot normally. it would bring much more depth to the shading.
same for skin reflection, could be more stronger and better defined.
the feathers on the wings needs some detailtextures which exaggerate the feather structure more.
and perhaps the deaths sense could be thinner and also more contrast in reflection for the sharp cutting area, like he's sharpen it 10 min ago. this would make it look more dangerous and deathly.

The idea of the image is fantastic, the angel and death playing with the old man's life, but technically, mainly the texturing and shading work has no life. I don't know if is your visual style, but to me there are things to improve.
When I see the image as a whole, the materials are identical, there's not much difference between clothing and skin materials, what makes them different is only the texture you've applied. The skins are much artificial, plastic and miss a SSS. Your zbrush renders are better than the final render. When I see the old man arm and hand, the skin specular/reflection looks like plastic and 3d, as well as the angel face.
Your modeling work, lighting and attention to details are great! But again, your materials killed the image.
