There is a video from Gnomon online that tells about this problem and how to solve it. And you know, it is free to. You can find it on this site http://www.gnomononline.com/character.html .

1. Parent your IK to a locator
2. orient constraint the bone where the IK sits to the locator
3. Test result

PS: IK solves position not orientation

You also need a locator or something to make a Pole Vector for each of the elbows.
I recommend making a locator for the Ik handle. and point constrain the IK to the locator. Then another locator for the Pole Vector constraint. Then you can hide the IK handle and only use the translations of the Locators to drive the arms.
