Hi JFFORTIN,
I do face the same problem here. I got to animate a snake, so I use IK spline with offset like you.The snake flip when it needs turn around. So I find some help in "help". Refer to it, you need to turn off "root on curve" and use "auto create root axis".(use your own curve, not genaral). Then it will create a transform node on top of your joint chain. "Attach to path" this node to your curve.
And forget the offset, use the path animation to drive your joint chain now. With this method. The snake won't flip now...but only in plane curve. If the curve is complex....It still flip,...too bad.
But Maya5's ik spline seems have problem..you can try it.
I've asked in here......but no one have answer.
I don't know if you know this yet or not...
anyway hope this help.

 First sorry for my bad English.
If I understand your question correctly.I think I can give an answer about this flip.
In maya5.0 if you enable Skeleton > Enable IKFK Control, then you'll find End effector doesn't move with your joints and IK handle. And this maybe the reson for flip.
when you disnable it ,maybe you can get you want just same as maya4.5.
If I misunderstand what your discuss, just forgive me.
