Most likely because the object isn't symmetrical, easiest way to test is to go to a front view and MMB loop select the right side of the object, if the left side doesn't select it isn't symmetrical

Yeah I had the same problems, just try to delete mirror side and then make mirror again to make it symmetrical.

I'm not sure why Modo does this though: loose symmetry for no reason it seems :/

I often run into this problem and it is a huge letdown. The only thing i dislike about the program. I am turning to zbrush to paint / sculpt now because of this :(
