Did you freeze transforms on the target blendshape? Blendshapes work with the shape node, so you can't freeze transforms after making the shape. Here's the order that you should do things in.

1. Set up your base mesh
2. Freeze transforms and delete history
3. Duplicate your mesh (NEVER freeze transforms on the duplicate mesh)
4. Modify the duplicate mesh to get the desired shape.
5. Create blendshape

Since you never freeze transforms on the target, you never bake the transforms into the shape node, which I'm pretty sure is what's causing your problem.

One way to fix it is to select the blendshape target, and then select the base mesh. Hold down x, and snap the meshes to the same point in space. With both meshes directly on top of each other, freeze transforms and then create the blend shape.

If the pivot point hasn't been changed on either mesh, then this method should correct the error that is making your blendshapes work incorrectly.
