i use poly box and cylinders %90 of the time. Mostly because you can get any shape out of them.

aswell as box's and cylinders i also use a singular polygon as a starting point too - usually only for humans though.... or body parts. usually block out all the major muscles like this and refine from there.

it really depends. if you model by polys then i would also start with a cube or a cylinder. with nurbs i don't tend to use the primitives very often. only the circle primitive i use to start shaping a different curve.
in generell primitives are good for specific types of models. (cartoon like ones e.g.). if it's going into the realistic modelling, primitives can't do a lot for you and you will always end up using curves.

Sphere for organics, Poly cube for mechanics...so much for that little rhyme LOL. I start with a Nurbs Sphere when building a character, and I use a Poly Cube when builing anything mechanical like a vehicle or somethin.

I usually start with a poly cube and then I smooth it once to round it out a tad..then work from there...
Of course...it all depends on the type of model...
Usually when doing enviorments, I do it all nurbs..so i start with a curve.
