can you make a quick sketch of what you're wanting to do???

As JD suggests, a sketch would help - in the meantime, maybe a look at the Bridge / Weld and Inflate pages at the linked site will give you some ideas (for the holes thro' a cyl problem)

For this you've basically got to create a face on either side of the object (must have same no of verts) - and use Face | Bridge - this will create a tunnel thro'.

Dimples / recesses - Extrude Normal, Inset + move, Bump come to mind - but it really depends on what you're trying to do :)

Disc recess first:

Create a default cylinder
Select end face
Apply Face | Inset -> 20% (say, press contrl for 10% steps)
Repeat inset op (press shift + D)

You've now got 2 annular 'rings of faces' + 1 large central circular face.
Select the middle / 2nd ring of (16) faces (select one, press I for select similar)
With these 16 faces selected, apply Face | Extrude Region -> Normal (or Y) and drag to required depth ('height' also possible)

For 'dents'

Undo the previous ops (or start on another cyl)
Select alternate faces on the same ring as before (8 faces now selected)
Apply Face | Extrude -> Normal (or Y) and drag as required.

Could also use Face | Bump on these 8 faces or use Face | Inset on the 8 faces and then move Y the resulting smaller faces.
