.hmm.. good question.. i would really love to know how you guys out there deal with this... but for me..( maybe this is the dumb way):shrug: i usually key the key camera movement first ..then i used auto-key mode and just manually offset the inbetween.so its like.. <press the "right" button a few times--> moves the camera a bit..--> then press "tight" button a few more times again......> :hmm:

If by 'handheld' you mean that the cam should have a kind of random jerky motion, then one way would be to use expressions.
eg you could create a null and apply an expression like:
null.kine.local.posy + (rand_0_1( 5 )) -0.5
to: null.kine.local.posy
and then parent the cam (or only it's target) to the null. This would give you a small random motion in the y-axis, and similarly setup for other axes if you wan't
I would just do it in post. I'd motion track a piece of footage that is handheld, then apply those frames to your animation. you might want to render a slightly higher resolution than you need though, so you don't get black bars dancing around the edges.
