If you want to make it from a flat image in photoshop use crazybump instead.

I can't get over how good this program is at pulling out 3d shapes from a flat image. I've been using it a while now and can't get over how effective it is :) The Nvidia plugin will just give you a really flat effect, good for certain things, but for rocks and stuff this prog really does the business.

From a mesh Xnormal is probably the best tool around, and it's free.

XSI has a good normalmapper, and 3ds Max's render-to-texture can also give good results.

Do you mean a bump or displacement map? usually you use a low and high poly model and the normal map tools in your application to create the map. You wouldn't really even need photoshop for it at all. Normal maps are multicolored bump maps that "project" the high poly detail onto a low poly model. I can't imagine drawing one successfully.
