Either colorize it red if it starts out b&w. That or what they normally do is shoot it color and then remove the color on everything except the lips. Both can be done pretty simply with roto of the lips all in a Compositor.

You draw masks around the lips and animate them frame by frame. If you do so on an Adjustment layer, you can apply effects to the layer and the mask controls which parts of the layers underneath it affects. This is pretty straightforward, it's just a lot of manual work as is all kind of rotoscoping.

you could also try pulling a key from the lips and inverting the matte, since usually lips are a different color than the surrounding skin. Or maybe a luma matte. Either would require a garbage roto.
