4.5- anything really-
just the options to do things like "poly to sub-ds" or poly to nurbs etc. arent even showing up in the menu.
just nurbs to poly

save your maya prefs file under a different name (so you can get it back if you need it) and then dump it
restart maya
I'v had my prefs file screw things up like this before

i think if I was still on OS9 (mac) that might work, but I`m on OSX and the only thing maya has that could be considered maya preferances are in two places:
the first:
applications/maya/application_support/scripts/start_up
its called: mayaPrefs.mel
so trashing it, only makes it so maya won`t start up.
the second:
Library/Preferences/AliasWavefront/maya/4.5/prefs
and theres a couple of them:
-pluginPrefs.mel
-userHotkeys.mel
-userNamedCommands.mel
-userPrefs.mel
-windowPrefs.mel
trashing these allows the program to still start up, but nothing is fixed.
Grrrr. someone please help. I`m totally stuck.

Im not sure how the maya enviroments are set up on OSX but I'm guessing there the same
you should have a prefs dir and in there are dirs for marking menus , icons ..... in there look for a userRrefs file
if its there dump that

theres nothing called "userRrefs" just "userPrefs" (i asume its a typo)-
