CGTalk - License Transfer

Can anyone tell me or point me to an example of what the license transfer piece of paper needs to read like? I'm selling a copy of Lightwave that's been registered by myself and I need to write something for the person purchasing it.

this is an excerpt of the mail I got from Newek

Please remind the customer that he needs to provide us with a "transfer agreement" . This hand written document must contains the names, adresses of the former licencee who declares to freely give the lce nrLW*******, version
*.* to your end user (full name and address must be detailed as well). For a perfect transfer, the document should contain both signatures.

I sold on old copy of lightwave (d'oh) that I had aquired a while back and registered but never used. All I did was put my info and his info on a peice of paper and said that I wish to transfer the license to this guy....

He said he was going to upgrade ASAP, never heard back it must have worked.
