You can make 3ds Max plugins using maxscript.
Go through the topic 'scripted plugins' in the maxscript help.

In case you referred to the more hardcore c++ thingy, then max sdk is the least you need
in addition to visual studio.

No (not C++ ones anyway) and no.

You can't, but it does come with the 3DS Max install DVD.... (you have to install it seperately from the main app)
