In Maya, you get the above result when you apply a material with a texture to unmapped geometry. I've not seen it in any other situation. You say that you sometimes get a correct render? that's odd. Is the geometry mapped? if so, you may have inadvertantly done something to the history which has removed the UVs, but i've never seen that happen personally. Did you delete your construction history and then run clean up or what?

Yea you get that effect on a model with no UV's. If you've inadvertently deleted them it's not a problem, just apply a new cylindrical map and it should behave normally.

in order to keep me out of trouble (delete the essential UV map for part of my mesh), I tend to label something unique.. For instance, "cylinderM" means "Material for Cylinder." By doing this I have less accident to delete what I should not delete... Proper lableing in 3d pakage is challenging but saving my life for sure... lol

Yea I would have to go along with that. It's also not a bad practice to start off with a mesh with no UV's (delete the predefined ones) so that you can easily see which parts you have mapped as you go along.
