ok, your nagging worked :P
just a few pointers on the head
make the chin wider, and the nose, also make the nose a little bit bigger
dont know maya yet so I cant help you with the other problems, sorry

i think the lack of replies might be because of two things:
1) people dont really know where you are planning to go with your model
2) maybe there is just not too much to crit
do you have any reference to show what you are trying to model ??
other than that: the nose and chin looks funny. maybe its on purpose, i dont know, but maybe you should consider widen his chin and straighten out his nose a bit. it looks kind of squashed.
accentuating the eyebrow are might also be a good idea to get some more character into him. hes looking a bit generic right now.

Have you tried to the Birail? You could (for that section) Draw curves on the existing surfaces to bring you up to a point, and then pull up a few CV's on your new curves. Put your original surface on a layer, and hide it, and then try to birail a path around, and you may have your lift if it all turns out.

EP curve tool, snap to curve to square in, or close in the ends then try a planar. I'm new to modeling, but that 's what I'd try. Then I would delete histiory on all attached parts, and group them together.

Nurbs modeling a head is so difficult...what you need to do to smooth out the head is probably just start rebuilding those patches. What I would do is draw some curves over the head you already made using snap to curve you could just make top down curves or mouth out curves or whatever. While you were doing this you will be simplifying those curves, so that you end up with cleaner geometery. This is really the best way, nurbs is always build and rebuild. You could experiment with using the artisan smooth tool (make sure your seams are stiched first or your they are going to get ripped apart). After words you are still going to have a busted seam in the middle, and you are going to need to pull the cvs back. Either way, its just going to require patience and some tedius work. I'd also recommend that you get the whole body roughed out a bit before you start working on the clothes. And for those two surfaces you wanted to attach: duplicate the isoparms at the end of each surface and loft between them. Then use the round tool to smooth the edges.

dunno if this has been said , but to close the gap I would select the first surface and select the isoparm on the top and then shift+select the other surface and select the isoparm on the other side and just make a loft...hope that was explained properly...if not then please say so
