I've seen blank templates at office supply stores. Just write them in and slap it on your keyboard.

Had to dig for this and I cleaned it up a bit with photoshop, but I think this is what you are looking for. I believe it's for Max 4 keyboard shortcuts so should translate to 5 ok.

They redid quite a lot of the shortcuts in max 5. qwer is select, move, rotate and scale respectibely. And that's just those four keys...

There is always the HotKey map. It has been updated and is in your 3dsmax5 root directory ( c:\3dsmax5\hotkey.html)
Its a flash, mouseover map. Open it up in a browser or viewport for reference. Run the pointer over parts of the keyboard to indicate what's there.
Copy it all to a new template for printing if needed.

I would not make a keyboard chart thats like looking at the keyboard when you type (ie slow) Make a list of the keyboard commands keep it in the car, take it with you on the bus. I keep my copy in the restroom, and just read them over and over. If you are looking to get fast having them memoried is much faster than looking at a handy list. It may take time to get there but it is well worth it just like properly learning to type.
I have a list of the 3ds Max 5 commands on my web site. All of the default commands are there, I also filtered out all of the commands that do not have shortcuts.
