Are that realistic textures made by hand from start to finish?!

Depends on what kind of texture it is. Stuff like concrete is almost never painted entirely by hand; it's usually based on a photo. But "some adjustments" doesn't do it justice. It often takes quite a bit of work to turn a so-so photographed texture into a good one.

Both techniques are used, although I'd say most stuff starts from scans and then gets heavily manipulated.
