This may not be as step-by-step but if you're looking for inspiration on title design some of the Rockport Press (http://www.rockpub.com/books.asp?book=4) books can be very enlightening.

Have you used Motion (http://www.apple.com/finalcutstudio/motion/) before? It makes an amazing companion product to Shake and has a great toolset for title design and real-time motion graphics work.
