polys or box modeling.
are they going to be part of the same mesh that are the fingers...
if yes then i would use box modelling and extrude them out of the fingers ...

they are gonna be with the mesh all together, but this is what happens:

post a wireframe of the low poly before meshsmooth and wireframe after meshsmooth...

I think you put crease on it or something.
Here's how I learned it. Basically box > meshsmooth (1) > convert to polys. Inset polys, move, inset, move. Extrude and move a couple of times. Add detail with selecting edgerings and connecting them. Move and scale vertices with soft selection on to get general shape, I skipped the last and most tedious part. But you meshsmooth and see where it needs tweeking and more details:
