instead of extruding it out of the arm..

how about using the snap tool and snap to vertex on your mesh.

in terms of a strap around an arm you could also draw a splin and extrude along it

hey dude,
by default the snap key is "s" .also right click on the snap icon at the bottom an the options pop up.
so if you need anything spline related then you could snap to the vertices of the mesh...and many other options.
also if youre using cs poly tools then you could build geometry with the spline you just made.again... check out cs poly tools and the documentation will make things clear.

You can try selecting the edges in edit mesh or edit poly mode and converting these to a shape. Works for most of what you will want to do. You can also select faces and copy them into a seperate element and extrude them. All depends on what you are trying to do.

You could also use spline pencil (http://maxres.cgworks.com/scripts/). It can draw splines on any surface.

One technique I like for making armor or straps or anything that has to be form hugging is to copy the skin (arm or body) apply a push modifier (this expands or contracts the mesh) and simply hack away the stuff you don't want and cap...
With max5 (or meshtools) you can just use the edit poly "cut" tool and carve right onto the surface then extrude (if you want the entire thing to be a single mesh).
