I'm a bit confused as far as whether to blendshape or smooth skin first?

Can someone pls enlighten me?

Smooth skin. Then, identify where your problem areas are and use blendshapes to clean them up.

I'm not sure I'm following you.

If I have face blend shapes for speaking and a skeleton with the body binded, do I just bind the base object head and then no matter where it moves it will deform?

I think what seven6ty ment is that u can use blendshapes to fix some problem areas, for ex. the shoulder area, when deforming with smooth skin... Now for your problem - there is no matter when will u assing the blenshapes - before or after smooth skinning (i think the common worflow is "skinning->blendshapes"), as long as the blendshape is BEFORE the skincluster in the deformation stack... Think of it this way - u get the facial deformation first, then u deform with the skincluster. If the blendshape i AFTER the skincluster it will override the joits deformation. Hope thats helps. :)
