Making different maps is very rarely a simple case of converting one to another. This is because you're left with a flat gray image that may not necessarily be appropriate at all.
Reflection maps for skin work simply to create areas that are more reflective than others. In this regard they serve the same purpose as specularity, since specularity is essentially a fake reflection. The problem with spec is that it makes skin look plastic-like. So using low levels of tinted reflection often yield better results.
All you have to do is study some photos or even your own skin and just identify which areas are more reflective than others. And then create your map accordingly. No big mystery or secret technique to it. Just paint what you see and what you need.
