Well, first of all, your description is pretty vague.
Secondly, I believe Meshtools has problems with being put in the Quad Menu, doesn't it? Check the documentation on that one. I use an icon toolbar myself.

Go to Customise Interface
First make a new menu...(you don't have to but it can be helpful)
Load all the Mesh Tools commands into that menu and save
Either with a new Quad or even the default one (I use the default one for ease) drag the menu onto the Quad and save.
Only a couple of commands are problematic on the Quad.. for those use the menu that you will now have at the top of the interface or construct a tool bar.
Hope this helps. :)

Actually
As far as I know, u can have all commands in quadmenu, the 4.2 update for max made that possible.

This is how i set it up, i just replaced the standard quad menu to the top right, never used those commands anyway.
Now u cant see all the commands, because Im in edge mode, and then u cant see the vert. or edge commands.
What I did:
customize ui, delete all the commands in that quad, and then just drag and drop the mesthools command, in an order ilike, (the most used at the bottom of the menu etc. no I odnt use the cspoly commands that much, so theyre moved up the list)
