this is definitly something for compositing. there is a nice little plugin from tinder available for shake and this works such great.

I believe a while back he did one on steam coming out of a vent, very applicable to this.

I had a go at doing this kind of thing a while back. What I did, was created a load of particles with instanced geometry attached to them. On the geometry which was attached to the particles, I had a transparent material with a high IOR and then left it to render - I got some reasonable results but, in the end i did it in post because it was very slow to render.

Yes take a look at DV garage that is the best way:
If I remember:
Use particles but don't worry about the rendering (use basic RGB values) just worry about motion and location.
Then load in after effect your particle pass and use it as a displacement layer for your footage. The saturation/hue of your particles controls the amount of displacement/blur.
Brilliant and ultra fast! I love DVgarage.
