Looks like the places that have holes, have got some of the verticles unmerged. So select all the verticles, go to "Merge Verticles option box" and put the treshold to 0.01 or so, and click "Merge", should work after that.

if you go into wireframe with the unsmoothed, check if there are any faces inbetween the places where it splits, if so, delete those faces and try again. Also merging verticles might help.

usually the face is still there ( inbetween ) so...try to delete the face inbetween...and smooth again..should work

for the next time:

check Polygons -> Tool Options Keep New faces together

This just takes part for the extrude tool :)

Believe me when I tell you this but I had the same problem too.. I was pulling my hair out but all you need to do is wherever you want to curve the box then you have to make more poly faces and then smooth them out. wherever you want it to look straight then just leave 1 poly face? If I am confusing you just look at the Maya guides under polygons and smoothing!
