how can i model the arms and shoulders with splines
or by any way
i tried spline patch
and mix of them but failed please help me if you can

The simplest way IMO is box modelling by extruding the arm from the torso. You must prepare the arm location in your torso. Usually 4 quads are enough to build a fairly detailed arm. Three to 4 smooth shifts properly placed are enough to build the shoulder. You will need a couple of spin quads to get a proper polygon flow.
After that the arm and forearm are made by smooth shift. Do not hesitate to increase the number of smooth shifts at the elbow area for better deformation (3 SM rows of polygons give great results).
What you will need is to tweek vertices as work goes on.

Why dont you post what you have so far?
It would be easier to see what youre doing and how to correct it.
